When you first arrive, we will ask you to fill in a medical history questionnaire. You can save yourself time by downloading the new patient form and completing it throroughly prior to your initial consultation.

Complete Medical Examination

• including orthopedic, neurological, muscular and chiropractic exam. The purpose of the examination is to confirm that your condition is in fact a chiropractic problem and that you can be helped at this clinic.
